June 2025: Connecting RSAT Participants to Community Care

Q: Our RSAT program is trying to improve continuity of care for participants after release, but we struggle to get community providers to follow up or even return our calls. What strategies are other jails and prisons using to build stronger connections with treatment providers in the community?
